Chronic Disease Care Program

 

In medicine, a chronic disease is a disease that is long-lasting or reoccurs periodically.  Chronic conditions are the major cause of illness, disability, and death in the United States.  The number of people in the United States with chronic diseases is rising.

 

 People living with chronic diseases need medical treatment from their health care providers but they also need support that will enable them to self-manage their health as much as possible. Our goal at MCCN is to improve clinical outcomes and quality of life for our patients with a chronic disease.  Utilizing a case management approach, each patient referred to the program is assessed and an individualized care program is established for them.  This care program is reviewed and updated periodically to assure that quality care is provided

Some of the chronic diseases that are treated in this program include: Anemia, Arthritis, Asthma, Diabetes, Dyslipidemia, Hypertension, Obesity and Osteoporosis among others.
