If you need help to complete the applicatin, MCCN can assist in completing this application. Please call 818-895-3100 to talk to our Certified Application Assistant (CAA) 8 a.m. to 6 p.m, Monday through Friday. These CAAs can help you fill out the application at no cost to you.

Who may qualify?
- Children up to their 19th birthday.
- Parents, stepparents, legal guardians, and caretaker relatives who live with a child who is enrolled in Healthy Families, no-cost Medi-Cal, or who has two months of accelerated Medi-Cal Coverge. For any child of group of siblings, only one caretaker relative or legal guardian may be eligible.
- Children in families with incomes at or below 250% of the Federal Income Guidelines.
- Parents, stepparents, legal guardians, and caretaker relatives with children in Healthy Families or no-cost Mdi-Cal and who have incomes between 100% to 200% of the Federal Income Guidelines.
- Families or persons in the family without employer-sponsored health insurance in the last 3 months.
- Families or persons in a family who are not eligible for no-cost medi-Cal.
- Parents, stepparents, legal guardians, caretaker relatives, and children who are U.S. citizens, nationals, or eligible qualified immigrants.
