CHDP Overview
The CHDP program is a preventive health program serving California's children and youth . CHDP makes early health care available to children and youth with health problems as well as to those who seem well.
Many children and youth in California have unmet health needs. Through the CHDP program, eligible children and youth receive periodic preventive health assessments. Children and youth with suspected problems are then referred for diagnosis and treatment. Many health problems can be prevented or corrected, or the severity reduced, by early detection and prompt diagnosis and treatment.
CHDP works with a wide range of health care providers and organizations to ensure that eligible children and youth receive appropriate services. These CHDP providers include private physicians, local health departments, schools, nurse practitioners, dentisits, health educators, nutritionists, laboratories, community clinics, nonprofit health agencies, social and community service agencies.

Health Assessment Services
At the time of the health assessment, CHDP providers make referrals to Women, Infants and Children (WIC) Nutrition Program for children to age 5; dentists for preventive or restorative care; and medical providers as necessary.

Dental Services Medi-Cal recipients under age 21 are eligible for dental services, including preventive dental care, through the Medi-Cal dental program known as Denti-Cal. These services are provided by dentists participating in the Denti-Cal program |

Care Coordination CHDP provides care coordination to assist families with medical appointment scheduling, transportation, and access to diagnostic and treatment services. |
CHDP School Entry Requirements All children entering first grade are required by California law to have either a certificate of CHDP health examination or a waiver of such on the file at the school in which they enroll.

| CHDP Gateway The Child Health and Disability Prevention Program (CHDP) will be a gateway to provide access for over one million uninsured children to the Medi-Cal or the Healthy Families Program through an automated pre-enrollment process.
